How to Request an IME?

If you find yourself needing an IME, you'll want to use Form LWC-WC 1015, titled 'Request for Independent Medical Examination'. This form can be downloaded, filled, and should be submitted to the Medical Services Section of the Office of Workers' Compensation Administration (OWCA).
